Can't handle on-line registration? Computer acting up on you? We understand--it's not your fault.

Here are the manual registration forms, which will cost you an additional $5.00 for manual processing. Forms must be completed legibly and fully. E-mail address is required to complete registration. (see link for free Hotmail account below.)

Junior Volleyball Players: Complete using on-line PDF & sign (parent, too) & bring to registration:

Junior Olympic Volleyball Player Medical Release Form
Liability Waiver and Code of Conduct
Grievance Procedure read it--this solves many problems before they happen.
Player/Parent Agreement-Emergency Release Form

Above forms are PDF "fill-in" forms so we don't have to read your lousy handwriting (and you know who you are.) Fill them out on your computer. In Adobe Reader, just "Select Text" then move your cursor. Print it out.

Be prepared to complete print all of these forms and take to registration with a check for $105.00 payble to "YMCA JOVB"
